Changes to NC-504 Coordinated Entry Services

As Partners Ending Homelessness (PEH) begins a pause in its operations on Wednesday, December 10, 2025, we are writing to share important changes regarding NC-504 Coordinated Entry (CE) services. This includes where assessments can be completed, how referrals will be handled, management of the community’s By-Name List, and how consumers will be able to access the Coordinated Entry Access Line and email communication system. These changes are being put in place to ensure that services remain accessible, consistent, and well-coordinated for individuals and families experiencing homelessness.

Access Points and Assessments

Main Public Access Points

Cultural Arts Center (Greensboro) – Mondays & Wednesdays, 8:30 a.m. – 10:30 a.m.
This is the most recognized and well-attended location for CE assessments.

High Point Public Library (High Point) – Tuesdays, 10:00 a.m. - 12:00 p.m.

Please note that Wednesday December 24, 2025 and Wednesday December 31, 2025 the Greensboro Access Point will be unavailable due to the holidays.

In addition to these locations, several community agencies have been trained and approved by the CE Committee to complete assessments for clients they serve. Please note that some agencies may only be able to assess individuals already receiving that agency’s services.

Access Line

Consumers can call NC-504 member organization Fox Valley Farms at 984-209-9709. If the voicemail is received, please allow up to 48 hours for someone to respond to your call. 
The hours of operation will remain 8:00 a.m. to 5:00 p.m., Monday through Friday. Voicemails may be left after 5:00 p.m. and on weekends.

The Coordinated Entry email will be unavailable for consumer use temporarily.

Assessment Submission Process

Moving forward, assessments will be sent to a new centralized email address through The Servant Center, ceassessments@theservantcenter.org.

Assessments will be saved and stored to ensure accuracy, timely follow-up, and transparency. File storage will be coordinated through the CA/CoC Lead, ensuring that all documents are properly managed and accessible to the CoC CE Committee Chair and Vice-Chair for oversight.

By-Name List Management

The community By-Name List and weekly meetings will be managed by The Servant Center with the responsibility of oversight by the CoC CE Committee Chair and Vice-Chair to ensure system accountability.
