Integration of services and data is the key to successful people in healthy communities. Guilford County strives to use people-centered data to allow policymakers and leadership to put individuals, not specific departments or programs, at the center of the conversation. The integration of data and services brings together in one place the capacity to answer the big questions of how to improve the health and well-being of Guilford County residents, not just treat symptoms of the problems affecting them.
Goals and Objectives
Guilford County Government oversees the county's largest share of health and human services work. Promoting integration within the County’s health and human services operations requires long-term investments and sustained commitment to establish a thriving county-wide data and service collaborative.
A collaborative of this nature requires a formalized cross-departmental and cross-organizational partnership focused on the following objectives:
Objectives
Improve the health, well-being, and resiliency of County residents by practicing data-informed decision-making, focusing on individuals experiencing the cyclical nature of crisis.
Develop and implement a plan to coordinate departmental services for efficiency and effectiveness.
Leverage data analytics to identify high-service users and develop policies and interventions to cease the cyclical nature of crisis.
Use technology infrastructure to integrate data sources from all levels of government and community-based organizations to promote data visibility with residents and community stakeholders to ensure human-centered design techniques and the development of a high-performing county system of care.
Maintain internal and external cross-system data partnerships to support collaborative governance and key initiatives.
