Guilford County (NC-504) Continuum of Care Forms

The Conflict of Interest Disclosure Policy. The NC-504 Guilford County CoC Governance Charter requires the Acknowledgment and Disclosure Statement to be submitted annually by each agency participating in the CoC. A hard copy of this form is available and should be sent to infoCoC@guilfordcountync.gov.

Guilford County CoC Annual Membership Form. This form should be submitted by all CoC member agencies/individuals before July 1 of each year.

Coordinated Entry Confidentiality Form: A signed copy of this form is required for participation in Coordinated Entry Case Conferencing (Coordinated Assessment Workgroup) meetings.

ESG Client File Documents and Forms: The North Carolina ESG Office has provided the forms needed for all of their funded programs. Please continue to check the ESG website, as the forms are regularly updated and/or changed. The NC ESG Office has indicated that these forms may be used for non-ESG programs as well.

NC HMIS Agency Privacy and Confidentiality Policy: The NC HMIS Agency Privacy & Confidentiality Policy should be posted in a manner consistent with the posting requirements that are found in the NC HMIS Operating Policies and Procedures (page 28). The policy is also required to be in each agency's HMIS binder, which is checked annually.

Greensboro Community Housing Solutions - Home Repair Program
Weatherization Issues: For clients with weatherization issues (lack of insulation, broken doors or windows, HVAC issues, problems that normally cause high utility bills), Piedmont Triad Regional Council (PTRC) may be able to assist.
Assistance for the Elderly: Piedmont Triad Regional Council (PTRC) has a home repair program for people that are 62 years old and older.
Community Housing Solutions also has a program called Aging Gracefully for the elderly with mobility issues. This program can assist with things like: installing grab bars, installing ramps, and/or widening doorways to accommodate wheelchairs.
