*Note: do not print screen the fillable form, it will not qualify as a valid form, it must be on the State absentee ballot request form. You must go all the way through the process to the State form.


*** If you wish to type in all your information (except signature) before printing your form, you may do so on the fillable form (except signature). After you have entered the required information, click print preview at the bottom of the page (DO NOT use the browser or menu print preview or print screen buttons, it will not be on the acceptable form). If you have included all the required information, it will populate into the State Absentee Ballot Request Form (if you have omitted any required info, you will get an error message and you will need to go back and add it and then click print preview again.) Once you have completed all the required information, reviewed for accuracy and clicked print preview, the information that you have typed will auto populate into the required State Absentee Ballot Request Form. Please review your information as it appears on the form (if you need to change anything, you will need to go back and make the necessary corrections and then repeat the process). After you are sure that the information is accurate, you must then print the State Absentee Ballot Request Form (there is a print icon at the top of the form) in order to sign it and then you may submit the form in one of the ways detailed below.

These requests must be received in the office no later than 5 p.m. the Tuesday prior to Election Day. Once ballots are available, ballots will be mailed within one to two business days after receipt of the completed request.

If a voter expects to be unable to go to the voting place to vote on election day because of the voter’s sickness or other physical disability, that voter or that voter’s near relative or verifiable legal guardian may make the request in person at the office after 5 p.m. on the Tuesday before the election but not later than 5 p.m. on the day before the election.
